What it is
Remediation removes growth and the material it has rooted into, then dries and re-reads the assembly before it is closed. Mold grows into porous material rather than sitting on it, so drywall, insulation and carpet that supported growth come out; non-porous surfaces are cleaned and kept. On a water job it is the stage that turns an unfinished drying into a finished one.
When it is needed
When an inspection has found growth, when a cavity opened for drying turns out to have it, when a smell returns after cleaning, or when a stain keeps coming through fresh paint. Cleaning that has to be repeated is the clearest sign the material underneath is still wet.
What it typically involves
The area is sealed and put under negative air so spores disturbed during removal stay inside the containment. Affected porous material comes out and is bagged. What remains is cleaned and treated. Then the assembly is dried and read to the same standard as any water loss, because remediation over a wet assembly puts the same growth back behind new drywall. Many states set a square-footage point above which the work must be done by a licensed remediator, and several require the person assessing to be independent of the person removing; the contractor works to whichever rule applies where the property stands.
