What it is
Cleanup of water carrying contamination: a sewer backup, a failed septic system, or floodwater that has been through either. It is its own service because the contamination sets the scope, not the volume. A small backup across a finished basement can mean more removal than a large clean-water loss, because what it touched cannot be dried and kept.
When it is needed
Any backup through a floor drain, toilet or tub, any septic failure that reached the building, and any standing water of unknown origin that has been there long enough to darken. Waiting makes it worse in a way clean water does not: the contamination keeps spreading through porous material as long as it stays wet.
What it typically involves
The affected area is sealed off from the rest of the house first. Porous materials the water reached, carpet and pad, insulation, drywall to a margin above the visible line, particleboard, come out and are bagged for disposal, because contamination goes into the pore structure and no surface cleaning reaches it. Hard surfaces are cleaned and disinfected. Then the space is dried and read like any other water loss, and the containment comes down only once the cleaning and the drying are both finished. The larger scope compared with a clean-water job of the same size is the water class doing that, not the contractor.
